Job Summary:

The Medical Record Analyst will be responsible for reviewing, organizing, and summarizing patient medical records to ensure accuracy, clarity, and compliance. The role requires strong attention to detail, medical knowledge, and the ability to transform complex medical documents into structured, concise, and actionable information for healthcare providers, legal professionals, and insurance companies.

Key Responsibilities:

• Review and analyze patient medical records for completeness and accuracy.
• Organize records chronologically and categorize them by type (physician notes, diagnostic reports, hospitalizations, therapy notes, etc.).
• Summarize medical records into clear, concise reports highlighting diagnoses, treatments, and key case details.
• Tag and index records with critical details such as dates, physician names, facilities, and specialties.
• Identify inconsistencies, missing information, or discrepancies within medical documents.
• Ensure all documentation complies with HIPAA regulations and company standards.
• Collaborate with RCM teams, legal professionals, and healthcare providers for information clarity.
• Maintain high-quality standards with regular accuracy checks and reviews.

Qualifications & Skills:

• Bachelor’s degree in Life Sciences, Pharmacy, Physical Therapy, Nursing, or a related medical field.
• 1–3 years of experience in medical records analysis, summarization, or healthcare documentation preferred.
• Strong understanding of medical terminology, clinical documentation, and healthcare processes.
• Excellent organizational and analytical skills.
• Proficiency in MS Office and medical record management systems.
• Strong attention to detail with the ability to manage large volumes of records.
• HIPAA knowledge/certification preferred.
